Participation Waiver:
Every reasonable precaution has been taken to ensure the safety of the events and the participants. I understand that there are inherent physical risks associated with school activities. I, on behalf of myself, my kid(s), my heirs, successors and assigns, assume all risks and hazards incidental to the conduct of these activities, also transportation to and from these activities, and any medical costs that may arise with respect to my participation. I do further hereby release, indemnify and hold blameless Play2Learn Education Ltd., the employees, instructors and supervisors of Play2Learn Education Ltd. I also have read and agree with the school “Payment Terms and Conditions”.

Payment Terms and Conditions
1. All students need to register and pay course fees prior to course study. Cheque should be dated the date of registration
2. The course fee (NO GST) should be paid by Cash, or Cheque made payable to “Play2Learn Education Ltd.” Or e-transfer to director@play2learn.ca If there is no address and telephone number on the Cheque, please write them down on the back of the Cheque. Please always well keep your receipts for tax purpose.
3. Please email the filled form and your proof of Payment to director@play2learn.ca
